Surest Health Plan

Surest is a $0 Deductible Health Plan that’s designed to make your healthcare simpler and more predictable. Instead of meeting a deductible before your plan starts covering costs. So whether it’s a primary care visit, a specialist appointment, lab work, or picking up a prescription, you’ll pay a set copay for most services. You can see the cost for care in advance through the Surest app or website, so there are no surprises after your visit.

With Surest, you have access to the UMR network of doctors, specialists, and hospitals. Preventive care is covered at 100%, and you can compare providers and treatment options based on cost and quality before making an appointment. This approach gives you more control over your healthcare spending and helps you make informed choices at every stage of life.

Office visit copays include:

Labor and delivery at a quality hospital or birth center can be as low as $900

The copay includes: 

  • hospital charges
  • OB
  • anesthesiologist
  • epidural
  • emergency C-section
  • baby’s stay (if discharged with mother)

Jeremy falls on his hiking trip and hurts his wrist. He's worried it's broken, so he heads to the ER.

The emergency room price is $650. 

The copay includes:

  • hospital/facility charges
  • attending physician
  • radiologist
  • X-rays
  • splint

During a soccer game, Mike goes down hard and injures his knee. After talking with his doctor, he is now considering a knee arthroscopy.

A knee arthroscopy at a quality hospital can be as low as $1,200.

  • The copay includes:
  • benign bone tumor removal
  • implanted osteogenesis stimulator
  • diagnostic X-ray
  • anesthesia
  • IV fluids
  • observation care
  • diagnostic lab
  • implantable DME
  • Copayment 
    A dollar amount that you pay when a medical service is received, regardless of the total charge for the service. The insurance plan is responsible for the rest of the total charge. 
     
  • Deductible 
    A fixed dollar amount during the plan year (January 1st - December 31st) that you pay before the insurance plan starts to make payments for certain medical services. There is no deductible on the Surest plan. The High Deductible Health plan has both an individual and family deductible. The family deductible must be met by a combination of family members. 
     
  • Coinsurance 
    A fixed percentage that the insurance plan pays for the medical expenses after the deductible amount is paid by the insured person. After the deductible is met, the insurance will pay a percentage (depending on the plan you choose); your share will be the difference (again, depending on the plan you choose) until the out-of-pocket maximum is reached. There is no coinsurance on either the Surest or High Deductible Health Plan.
     
  • Out-of-Pocket Maximum 
    You will be responsible for paying until you reach the specified out-of-pocket maximum. Once the out-of-pocket maximum has been reached, the insurance plan pays 100%. For the Surest plan, the in-network out-of-pocket maximum is $5,000 single and $10,000 family.
     
  • In-Network 
    In-network means that your provider has negotiated a contracted rate under the United Healthcare Choice Plus network. The United Healthcare Choice Plus network is a national network with providers located in every state, and is NOT limited to those providers located in Colorado.
