  1. Mandatory 401a

    All benefit eligible employees are enrolled in the Mandatory 401a plan. This cannot be increased, decreased or stopped.

  2. Lifestyle Education

    All family members that are enrolled in medical get the $500 Lifestyle Education credit.

  3. Where do I find my insurance cards?
  4. What is considered a qualifying event for benefit status changes?

    Some qualifying events include:

    • Marriage
    • Divorce
    • Birth
    • Dependent gains coverage through a new job
    • Dependent loses coverage due to termination of employment
  5. When is Open Enrollment?

    Open Enrollment will be held during the last 2 weeks in October.

  6. Colorado in Motion (Physical Therapy)

    Your first 8 visits with Colorado in Motion are free, then it’s a $25 copay.

  7. Behavioral Health Benefit - Open Network
    • It’s a $25 copay whether the provider is in or out of network
    • Providers have a choice to bill or not bill the insurance, so you may have to pay in full and submit your claim to Meritain for reimbursement. Instructions on how to submit a claim can be found here.
  8. Urgent Care - Open Network
    • It’s a $50 copay whether the provider is in or out of network
    • Providers can choose to bill or not bill the insurance, so you may have to pay in full and submit your claim to Meritain for reimbursement. Instructions on how to submit a claim can be found here.
  9. Emergency Room - Open Network
    • It’s a $200 copay, whether the provider is in or out of network.
    • Providers can choose to bill or not bill the insurance, so you may have to pay in full and submit your claim to Meritain for reimbursement. Instructions on how to submit a claim can be found here.
  10. Do my children need to be enrolled in dental in order to utilize the Right Start for Kids (RS4K) program?

    Yes, in order to benefit from the Right Start for Kids program (where oral care is covered at 100% up to age 12), your children must be enrolled in Dental coverage.

  11. I'm having a baby.

    If you have a baby, you MUST fill out the benefit change form to add the new baby to insurance. Babies are NOT automatically added to your coverage.
