Colorado created an official withholding certificate in 2022 called Form DR 0004.  This new withholding certificate is not meant to completely replace IRS form W-4 for Colorado withholding, but to help employees in a few specific situations fine-tune their Colorado withholding. 

  • If an employee wants to adjust their state withholding taxes, the employee should go to UKG under Myself>Pay>Income Tax.  To the right of the screen the employee can click on Add/Change Withholding Form(W-4).  The employee should click on the Colorado DR 0004 form to adjust their taxes electronically. 
  • An employee is not required to complete form DR 0004 and can leave their taxes following the Federal Form by opening the Colorado DR 0004 form and then sign and save. The employee’s withholding will follow what the employee has selected for the Federal W-4 form.
