This flowchart explains how your health and substance use information is shared within the Larimer Integrated Network of Care (LINC).

Essentially, LINC helps connect your care across different services, and you decide what information is shared and with whom by checking boxes on the form.

This chart explains how your consent determines who outside of your primary medical/mental health team can see and use your health information, specifically for Health Information and Substance Use Treatment services, as part of the LINC program.

When you sign the consent form, you have options (Box #1, Box #2, Box #3) that allow you to control the disclosure of your records.

1. Health Information
   Your Health Information records are handled in two ways:
   • No Box Checked (Clinical Staff): If you check NO box, the Clinical Staff (doctors, therapists, nurses) can still share your data only as allowed by HIPAA (standard privacy law). Shared with:
     • SummitStone Health Partners
     • Jail Medical/Mental Health
     • Community Justice—Treatment
   • Box #1 (Non-Clinical Staff): Checking Box #1 allows Non-Clinical Staff (like case managers) to access your information for Referral, Support, & Threat Reduction Purposes. Shared with all clinical staff, plus:
     • Community Justice—Case Managers
     • Sheriff's Office—Co-Responders
     • Adult Protective Services—Case Workers
2. Substance Use Treatment Information
   • Box #2 (Clinical & Non-Clinical Staff): Checking Box #2 allows both Clinical and Non-Clinical staff to access your information for Referral, Support, Threat Reduction Purposes, AND Treatment, Payment, & Healthcare Operations. Shared with:
     • SummitStone Health Partners
     • Jail Medical/Mental Health
     • Community Justice—Treatment
     • Community Justice—Case Managers
     • Sheriff's Office—Co-Responders
     • Adult Protective Services—Case Workers
   • Box #3 (Clinical Staff ONLY): Checking Box #3 allows Clinical Staff ONLY to access your information for Treatment, Payment, & Healthcare Operations ONLY. This option limits access to non-clinical staff. Shared with:
     • SummitStone Health Partners
     • Jail Medical/Mental Health
     • Community Justice—Treatment
