Larimer Health is trying to understand and track the spread of COVID-19 in Larimer County.  With limited tests available across the country, we know our positive case numbers represent only a small amount of COVID-19 in our community.

How is this different from the symptom tracker? You need to have had a provider visit (virtual is okay) to fill this form out. This is to try and get an idea of COVID-19 provider diagnosed cases in our community.
