Need a copy of a Larimer County Department of Human Service record or document?  Please select the appropriate division and follow the steps to request the documents.    

Records may not be released immediately upon request, and depending on the complexity of the record requested, can take 30 business days to process. Please submit your request form as early as possible to avoid delays.

NOTE:  Law enforcement and Human Services agencies must submit a request for records on their official letterhead or on the appropriate form and must include the names and dates of birth of the children/family for which they are seeking records.  Requests should be emailed to recordsrequests@larimer.org.  

 

PLEASE NOTE:    The release of Adult Protective Services (APS) records will only be considered when the requestor provides an appropriate court order pursuant to CRS 26-3.1-102(7)(b). If the requestor fails to attach the appropriate court order authorizing release, the request will automatically be denied.

What You Need to Know: 

  1. Larimer County  is committed to providing you with records in an expeditious and courteous manner.
  2. The regular business hours for release of records is Monday through Friday 8:00 a.m. to 4:00 p.m.
  3. Larimer County considers the confidentiality of its records a priority under federal and state laws but recognizes the right of access to the records.
  4. All requests for copies of Larimer County records must be submitted on the APS Records Request Form filled out completely, and signed. The form can be:
    1. Mailed to Records Requests, 1501 Blue Spruce Drive, Fort Collins, CO 80524
    2. Dropped off at 1501 Blue Spruce Drive;
    3. Or emailed to RecordsRequests@larimer.org.
  5. NOTE:  If you mail or email the Records Request form, you MUST include a legible copy of your Driver's License or Photo ID.
  6. To better serve you, please provide a description of the requested records.  Be as detailed as possible.  
  7. Your request will be processed in the order it was received. 
  8. Once your request has been processed, you will receive a call or an email indicating when the records are ready to be picked up and the cost for obtaining the records.  ​​​​​​ If you live out of state, you can mail in your money order for the amount owed and we will mail the documents to you. 
  9. Larimer County charges $.25 per page for copies of all records.  All fees must be paid before records will be released. 
  10. We DO NOT accept debit or credit cards at this time.
  11. All fees must be paid by exact cash or money order (made out to Larimer County.)
