Pregnancy and the first year after birth are important times for both parent and baby. For people affected by opioid or other substance use disorders (OUD/SUD), getting consistent, coordinated care can be challenging. Barriers like complex systems, stigma, and unmet health and social needs can make it harder to access support.
The Perinatal Nurse Support Program connects pregnant and postpartum individuals with a dedicated nurse who provides ongoing, relationship-based support. Nurses help individuals access prenatal, postpartum, and substance use care; coordinate services across providers and community programs; and support each person’s goals for health, recovery, and stability.
What We Provide
Services are tailored to the needs of each family and include one-on-one home visits with a dedicated nurse, care planning (including Plans of Safe Care), and connections to resources such as mental health care, substance use treatment, parenting support, and basic needs.
By strengthening support during pregnancy and early childhood, the program aims to improve health outcomes for parents and babies, support long-term recovery, and promote safe, stable, and nurturing environments for young children.

- Perinatal individuals and families affected by opioid and/or other substance use disorders (OUD/SUD)
- People navigating recovery while caring for a new baby
- Those who would benefit from coordinated care across providers and community services
Enrollment is available during pregnancy and up to 6 months after birth. Families can continue in the program until the child’s first birthday.
