As responsible stewards of taxpayer dollars, it is incumbent upon Behavioral Health Services staff to ensure that funding opportunities are vetted as much as possible.
Required Application Documents
- W-9 (2018 form or later)
- IRS Public Charity Designation letter or EIN verification letter
- Organization's current fiscal year operating budget
- Organization's prior fiscal year income/financial statement (P&L) *full year, not partial year
- Organization's prior fiscal year balance sheet
- Most recent audited financial statements (if applicable)
- Note: Documents are stored in your organizational profile in the grant portal. Please ensure documents are updated with the most up-to-date information.
Our goal is for applicants to have the best information and tools to support their submission of a competitive application.
- Reach out to our team at any point. The earlier you do so, the more time we have to guide you through the process and answer questions.
- Attend a webinar so you feel best equipped to apply. Although attendance is not required, it is strongly recommended.
The number of applications has increased each year. Go to the Grant Portal and complete your application. For tips, refer to our

- In past grant cycles, applicants have unknowingly created multiple versions of applications.
- The key to avoiding duplication is to open a new application, hit "Save and Refresh" before doing anything else, and then add content.
