Step 2  

What do I need?

  1. Applicant and any living parent named on the applicant’s birth certificate present at the appointment or a notarized, signed consent form (DS-3053)

     

  2. Completed application (Form DS-11):
    1. Complete your child’s application on the US Department of State website and print it
    2. Paper applications are available in our office, but you must arrive at least 15 minutes early for your appointment if you plan to fill out forms in the office
  3. Evidence of the child’s citizenship:
    1. Original or certified copy of birth certificate, certificate of naturalization, report of consular birth abroad
          - OR -
    2. Previous passport (if applicable)
      1. If the child’s previous passport was lost or stolen and had NOT expired, please visit the US Department of State website and complete the section for Report Lost or Stolen. Print the DS-64 form after completion and bring it with you to your appointment.
  4. Proof of relationship to child: You must provide proof of parental relationship to child
    1. Birth certificate (U.S. or foreign)
    2. Adoption decree (original or certified copy)
    3. Divorce decree/parenting plan granting sole decision-making or permission to apply for the child’s passport (original or certified copy)
    4. Court order granting sole legal custody or giving you permission to apply for child’s passport (original or certified copy)
    5. Please see the US Department of State’s webpage for guidance when the legal guardian of a child is an institution.
  5. Parents’ Consent
    1. Any parent(s) listed on the birth certificate must provide consent for a minor child to apply for a passport
      1. Parent(s) must appear in person with the child

        - OR -

    2. Provide a notarized, signed consent form (DS-3053) from the parent not present
      1. A notarized, signed consent form (with the wet signature) must be accompanied by FRONT AND BACK photocopy of ID presented to the notary
    3. Death certificate (original or certified copy)
    4. Court documents indicating sole-decision making (original or certified copy)
  6. Identification: Physical government-issued photo ID for parents/legal guardians. Acceptable forms of ID include:
    1. Valid (non-expired) driver’s license or state-issued ID card
    2. US or foreign passport
    3. US permanent resident card
    4. If your ID is a non-driver state-issued ID card or instruction permit, or if your driver’s license is from a different state, please provide a second form of ID such as a student ID, Costco card, gym membership, etc. (preferably a photo ID).
    5. A complete list of acceptable forms of identification can be found at the US Department of State website.
  7. A passport photo can be taken during your appointment for $15. You can also have the photo taken elsewhere (Staples, FedEx, Walgreens, etc.) and bring it with you to your appointment.

How do I pay?

Two separate payments are required:

  1. Check or money order payable to the U.S. Department of State. This fee must be a personal check, money order, or cashier’s check. Starter checks are not accepted (name and account number must be imprinted on the check).
    • Check the following boxes to calculate the total to the U.S. Department of State:
      • Passport book (valid for all forms of travel): $100
      • Passport card (only valid for land and sea entry from Canada, Mexico, the Caribbean, and Bermuda): $15
      • Routine service, no additional charge. Current processing times are available on the US Department of State's website
      • Expedited service (For the most current processing times, please check the US Department of State's website): $60
      • Expedited return shipping (passport in hand, 1-2 days after it is mailed): $22.05

  2. Payment to Larimer County Clerk & Recorder. Check the following boxes to calculate the total to Larimer County:
    • Execution fee payable by cash, check (made out to Larimer County Clerk), or credit card (3.5% processing fee applies to all card transactions): $35
