The convenience of submitting your passport application is available at the Larimer County Recording Department located in the Larimer County Administrative Services building at 200 West Oak Street in Fort Collins. We are designated as a Passport Acceptance Facility. All necessary forms are available, and our trained staff can answer your questions. We offer photo services for U.S. passports or visa photos.

Passport Books

A United States passport book is an identity and citizenship document allowing United States citizens to travel internationally and return stateside. It may be used by U.S. citizens to travel by any means to any international travel destination.

Passport Cards

The passport card may only be used by U.S. citizens to re-enter the United States at land border-crossings and sea ports-of-call when arriving from Mexico, Canada, the Caribbean, and Bermuda. It cannot be used for air travel to any of these destinations or for other international destinations.

Passport Fees

Current routine processing time is 4-6 weeks. Expedited service is 2-3 weeks.

Complete and submit Form DS-11 to a Passport Acceptance Facility. Forms are also available in our office. Please print single sided, completed in BLACK INK only.

Other Items Needed:

  • Certified copy of birth certificate or Certificate of Naturalization and a black and white photocopy of the relevant document. If the certified copy of the birth certification cannot be photocopied because it is printed on security paper, a second certified birth certificate will be required which will not be returned. Color copies are not accepted.
  • Government issued, photo ID
  • Passport photo (Unless medically necessary, glasses are no longer allowed in passport photos.)
  • Fees - The portion of the fees paid to the U.S. Department of State must be in the form of a check or money order. Cash or credit cards cannot be used for this fee. An Agent Acceptance Fee is payable to Larimer County. This fee can be paid in cash, check, or by using a credit/debit card (with a small convenience fee added for the electronic transaction).
  • If the applicant is under 16 years old, both parents and the applicant MUST be present at the time of application.

  • Previous passport
  • Photocopy of birth certificate (Photocopies should be black and white only – color copies are not accepted.) If the certified copy of the birth certification cannot be photocopied because it is printed on security paper, then a certified copy must be sent which will not be returned.
  • Government issued, photo ID's for parents
  • Passport photo (Unless medically necessary, glasses are no longer allowed in passport photos.)
  • Required fees
  • Both parents and the applicant MUST be present at the time the application is submitted.

  • Required fees
  • One parent and the applicant MUST be present at the time the application is submitted OR a 17 year old may appear without a parent IF a check is drawn from and signed by a parent.

Complete and submit Form DS-82 by mail to the address provided on Page 2 of this form directly to the U.S. Department of State. Forms are also available in our office. Please print single sided, completed in BLACK INK only.

  • Passport photo (Unless medically necessary, glasses are no longer allowed in passport photos.)
  • If a name change has occurred, include a certified copy of the legal name change document, i.e., marriage license, divorce decree or court-ordered name change document.
